Real Examples of Individualized Treatment Plans

The treatment plans below are from clients in the Treatment-in-Residence or Visitor’s Program.

The Reunion San Diego schedule includes two to three individual sessions per day. The Men’s Residential Program allows for additional individual sessions as an optional expense. Regardless of the facility or program, all Practical Recovery clients or residents collaboratively create with our professional staff a problem list and treatment plan that address their unique concerns.

The problem lists are in the client’s own words, and are not listed in the order of importance (which often changes). The lists and schedules only hint at how time in session was actually spent. The problem list is just a starting point. By the time you leave, we want you to have a solutions list!

We use “therapist 1,” “therapist 2,” and so on, so you can track with whom sessions are spent. The proportion of time with each therapist may change as the treatment plan collaboratively evolves. To remind you, after you have experienced the members of our staff, you get to choose to work with the ones you find most helpful. If you have been to treatment before, you may have experienced being “stuck” with a staff member you did not find especially helpful (if you got to have one-on-one sessions at all!). Although each staff member is a highly trained and dedicated professional, the nature of human interactions means that you will connect with some individuals more than others. We want you to have the “best of the best” while you are here.

What you are seeing below is the final schedule. In the planning process the schedule may have changed, as we collaboratively adjusted it to make maximum progress, and to allow the client to work with staff of his or her choosing. 48 hours notice of schedule changes is required. Schedules are normally constructed on a weekly basis.

Self-study time is used for homework, reading, watching educational videos, journaling, or other relevant activities as mutually decided. Self-study time can provide a break from the intensity of individual sessions. These schedules also do not reveal the enjoyable and productive ways that our clients spend their free time. A portion of your session time is spent planning how free time will be used. Most clients complete some homework each night.

Notice the large amount of individual session time. Groups are not required at Practical Recovery (ask us if you are concerned about groups while in rehab). For most of our clients the individual sessions are the heart of the program, and one of the primary reasons they choose to attend Practical Recovery. Identifying information has been removed for each client.
