Problem Lists and Schedules:
Real Examples of Individualized Treatment Plans
These treatment plans are from clients in the Visitor's program. The problem lists are in the client's own words, and not listed in the order of importance (which keeps changing!). The lists and schedules only hint at how time in session was actually spent. The problem list is just a starting point. By the time you leave, we want you to have a solutions list!
What you are seeing below is the final schedule. In the planning process the schedule may have changed many times, as we collaboratively adjusted daily to make maximum progress on the solutions list, and to allow that client to work with the staff members of his or her choosing.
Self study time is used for homework, reading, watching educational videos, journaling, or other relevant activities as mutually decided. Self study time can provide a break from the intensity of individual sessions. These schedules also do not reveal the enjoyable and productive ways that our clients spend their free time. A portion of your session time is spent planning how free time will be used. Most clients complete some homework each night.
Notice the large amount of individual session time. For most of our clients the individual sessions are the heart of the program, and one of the primary reasons they choose to attend Practical Recovery. Almost all other treatment programs have group sessions as their core service.
